South Tulsa Ear, Nose and Throat Center utilizes electronic medical records. Patient registration is a three step process. First, patients should complete the registration forms which detail general demographic information. Second, patients should request a login to our secure portal. Third, the patient completes their patient health history on line through the secure portal
Step 1: Registration forms Download the registration forms.
In addition to your health history we have registration forms to be completed prior to your visit. These forms should be printed out and completed prior to your visit.
Step 2: Request Login
Step 3: Complete patient health history
This information is collected via a secure registration system that enters your patient health information directly to our medical records system. Completing this online questionnaire will take 5-10 minutes. In order to have access to the health history you must request a user name and password.
