Surgery Scheduling and Pre-Certification Instructions
Surgery Scheduling
Requests for specific dates for surgery should be made immediately. Our physicians perform surgery at many different facilities and scheduling is usually done well in advance. We make every possible effort to accommodate requests.
The process of pre-certifying and scheduling your surgery usually takes 5 business days depending upon your insurance company. Please allow 5 days before calling the office after you have given us your preference of surgical dates.
Any paperwork (FMLA or Disability) required by your employer/school for absence must be in our office 5 days prior to the surgery date. Charges apply.
Financial Obligations
Prior to scheduling your surgery we will check with your insurance company for information regarding the type of and extent of your insurance coverage.
We require a 50% deposit of your portion of the estimated surgical fee prior to scheduling your surgery.
If you have a patient balance after a claim has been submitted to your insurance plan, we will expect payment in full after you receive a bill. Accounts that fail to pay their balance will be subject to collection procedures and those costs will be added to your account balance.
Additional bills you will receive: You will receive a separate bill from the facility where you have your surgery. Additionally, you will receive a separate bill from the anesthesiologist. Many facilities require payment prior to surgery. Lab tests, cardiology clearance, chest x-rays will all be billed separately by the laboratory providing the service.
Pre-operative and Post-operative Appointments
Some facilities or surgeries require pre operative appointments at the facility prior to your surgery.
The physician will want to see you back in the office after the surgery one or more times depending upon the surgery performed. We will make this follow up appointment when the surgery is scheduled.
